As a Care Coordinator, you will play a vital role in supporting our Case Management team and ensuring each client receives exceptional, personalized care. You’ll help set up new cases, conduct in-home visits, and coach our caregivers — all while representing the heart and mission of Assured Quality Homecare.

Care Coordinators will be expected to understand the healthcare professionals you will interact with while assisting in coordinating your client’s care. You will work under the supervision of the Case Manager.
As A Care Coordinator, You Will Be:

Client Start-Up Support
- Conduct home safety evaluations
- Introduce caregivers and review care plans with clients
- Ensure all client/caregiver needs and materials are in place
- Introduce our agency to physicians when necessary

Ongoing Field Support
- Make weekly home visits to evaluate care and safety
- Update in-home materials and deliver needed supplies
- Coach caregivers and provide feedback
- Communicate regularly with client families and document all visits
Requirements Experience/Training:
- Speaks, reads, and writes English fluently and follows direction.
- Establishes and maintains caring relationships with care recipients and their families even in difficult circumstances.
- Establishes professional relationships with staff in medical and nursing facilities when applicable.
- Maintains effective communication with Supervisor and other caregivers through the regular use of a personal cell phone, email, and face to face interactions.
- Is considerate of care recipients and hold their information as confidential.
- Follows HIPAA and OSHA regulations as prescribed by ASQ.
- Demonstrates excellent interpersonal skills with tact and diplomacy.
- Handles multiple initiatives concurrently.
- Prioritizes job tasks based on business needs
- Problem solver with a strong attention to detail.
- Resolves conflict in a direct, positive manner
- Fosters positive dialogue and feelings toward the company, coworkers, and the tasks being managed.

- Utilizes company electronic computer software programs effectively. Has a working knowledge of Microsoft Windows, Microsoft Word, Excel, and PowerPoint.
QUALIFICATIONS
- Must have an Active Certified Nurse’s Assistant Certification with CT
- Must have a valid drivers license, reliable transportation, and liability insurance
- Must have 5+ years experience as a caregiver
- Compassionate, Honest, Dedicated, and Reliable
Compensation: $21.00 - $23.00 per hour
Believe it or not, our story begins when our CEO Samanta (Sam), a hospice registered nurse, was challenged by her husband, Caleb, to "Do something about it!"
Working as a hospice nurse caring for the elderly with terminal illnesses, Sam's greatest frustration was that the caregivers hired by her patients' families had no idea what they were doing. It negatively affected her patients' well-being and comfort. After investigating the matter, she found that Connecticut set no standards for home care agency owners or the caregivers they hired to care for others. As a result, anyone could be employed as a caregiver with little to no experience, training, certification, or supervision, and this was often the case and still is today.
After weeks of complaining about this to her husband, being the no-nonsense type of guy he is, he challenged her to "do something about it." Little did he know that she, in turn, would convince him to leave his engineering career and help her start a home care agency, providing the elderly with high-quality caregivers who would keep them well cared for and safe at home.
Since then, Sam has continued to share her vision with countless other professionals. Convincing them to leave their careers and join her fight to be the Gold Standard for home care helping the elderly age in their home's comfort as they so often prefer.
